In order to be considered for placement, an application and Patient Review Instrument (PRI) must be completed. If you or your family member are in the Hospital, please ask the discharge planner to fax the PRI directly to St. Patrick’s Home for review. If you or your family member are currently at home, the PRI can be completed by a Home Care Agency / Registered Nurse.
